Applications for admission to Hale Academy can be downloaded from the PDF files below. Select the appropriate grade level, print the Application and the attached School Record Form. Complete all sections of the Application and sign the School Record Form. (Note that part 2 of the Application for grades 5 through 12 is to be completed by the applicant.) Please call to schedule an appointment to meet with the Head of School or Director Student Services at 352-854-8835.
During your scheduled appointment you will have an opportunity to become better acquainted with the Academy. If both parties choose to move forward with the application, a time will be arranged for the required testing and interviews.
